Nurse Care Manager Coordination
A hospice nurse is the main link to hospice care for your loved one and family. The nurse assesses what your loved one needs and then coordinates those services, including arranging for all necessary medical equipment and supplies.
The hospice nurse care manager arranges for equipment such as a bedside commode, lift chair, oxygen concentrator, linens for a hospital bed, or incontinence supplies. They also coordinate medication delivery and ensure that all supplies are readily available when needed.
Our care managers maintain ongoing communication with equipment providers and pharmacies to ensure seamless service. When equipment is no longer needed, they arrange for prompt removal, and when needs change, they quickly coordinate adjustments or additional equipment.
