Eligibility Requirements
To receive hospice care services, patients must meet Medicare eligibility criteria, including a physician's certification of a terminal illness with a prognosis of six months or less if the illness runs its normal course. We serve patients in Cook, DuPage, Lake, and select areas of Kane County.
Patient and Family Responsibilities
By accepting our services, patients and families agree to:

- Patients are responsible for any co-payments, deductibles, or non-covered services
- Financial assistance may be available for qualifying families
- Payment arrangements must be established prior to service initiation
- We reserve the right to discontinue services for non-payment after appropriate notice
