Heart to Heart Hospice was established in 2022 with a simple but powerful mission: to provide the kind of compassionate care we would want for our own loved ones. As experienced registered nurses, our founders recognized the need for more personalized, family-centered hospice care in the Chicagoland area.
Having worked in various healthcare settings, we witnessed firsthand how meaningful it is when families receive care that truly honors their loved one's dignity and wishes. This inspired us to create an organization where clinical excellence meets genuine compassion.
Today, we're proud to serve families across Cook, DuPage, Lake, and Kane counties, always staying true to our founding principle: treating every patient and family with the heart and care we'd provide to our own.
