Professional Social Work Assessment

Social workers are key members of our hospice team who assess the family's emotional, social, spiritual and financial needs to provide counseling and connection to support resources. Our licensed social workers bring specialized expertise in family dynamics, community resources, and crisis intervention.

As part of our interdisciplinary team, social workers collaborate with nurses, physicians, chaplains, and other team members to address the comprehensive needs of patients and families during this challenging time. They provide both direct support and coordination of community resources.
